How the heck did Denny Hamlin lose a valve stem out of his rim?

If something rubbed that hard on the inside lip of the rim wouldn't it have scratched up the rim, or gouged the tire also? I'm thinking the guy who put the valve stem in, didn't seat it properly. What say you?
